Absence Forms

If you are requesting to be absent from a game day or required performance, you need to speak with Connor Challey in person or at connor.challey@ndsu.edu

If you need to miss a specific day of practice, please full out the form below.

Please also let your respective section leaders know about any potential absences.

Any Band Camp absences must be approved through Connor Challey, please email him at connor.challey@ndsu.edu!

If you have a consistent class conflict with band that might cause you to be late to rehearsal (aka: "I may be late some days or I may be a few minutes late because of another class.") If you are filling this out to notify us about any short term absences, please full out the absence form above! DO NOT fill out this form if you have a class that overlaps with MUSC 111 and causes you to not be able to register for band! If you do have that conflict, please contact Connor Challey for more information!

If your class overlaps with band by MORE then a few minutes, you NEED to get a class conflict form from Connor Challey!!!
